This page tells you what research can prove. It can measure soreness, movement, and scan changes. Those results don't always agree.

You may feel better without a scan change. A scan change won't always ease soreness. Regeneration means care intended to support tissue repair.

## What result matters for my daily life?

Choose one action you miss. Maybe you want easier sleep or reaching. That'll give the visit a clear target.

Pain scores show whether soreness changed. Motion tests show how far you reach. Strength tests show what your arm can hold.

A scan answers a different question. It shows tissue wear at that moment. It doesn't measure your daily limits.

## What does the evidence say about PRP?

PRP, short for platelet-rich plasma, is one blood portion separated by spinning. That process gathers more platelets into the prepared portion. Platelets are tiny blood parts used in repair.

Knee research can't prove what helps shoulders. Shoulder problems won't always have the same results. That's why one promise can't fit everyone.

After an exam, QC Kinetix offers talks about regenerative care, which uses a prepared blood portion and aims to support repair. Improvement would mean less soreness or better use. It wouldn't prove that cartilage returned.

## What questions keep the choice clear?

Ask which shoulder tissue seems sore. Then ask what change you might notice. A useful answer names soreness, reach, or strength.

Ask how long it'll last. You'll also need the likely risks. The clinic should explain its exact blood preparation.

Compare those answers with your other choices. That includes exercise and surgery when needed. Cost matters when care isn't covered.

Write down the task hurting now. Check that same task after care. You'll have a fair way to compare.

The RESTORE trial - a participant-, injector- and assessor-blinded RCT of 288 adults aged 50+ with symptomatic medial knee OA (Kellgren-Lawrence 2-3) - compared three weekly intra-articular PRP injections against saline placebo, with co-primary endpoints of 12-month knee pain and medial tibial cartilage volume on MRI. PRP did not beat placebo on either. It is the single best-designed test of the specific claim that PRP changes joint structure, and it was negative.

2. A four-arm, multicentre, single-blind phase 2/3 randomized trial of 480 knee OA patients (KL II-IV) compared autologous bone marrow aspirate concentrate, autologous adipose stromal vascular fraction and allogeneic umbilical-cord-tissue mesenchymal stromal cells against a corticosteroid injection control. At 12 months NONE of the three orthobiologic injections was superior to another, or to the corticosteroid control, and none of the four groups showed a significant change in MRI osteoarthritis score from baseline. No procedure-related serious adverse events occurred.

3. FORWARD, the longest disease-modifying osteoarthritis drug trial reported to date, gave intra-articular sprifermin (a recombinant FGF-18) or placebo to knee OA patients and followed 378 of them for 5 years. Sprifermin produced a significant, sustained dose-response INCREASE in total femorotibial cartilage thickness versus placebo - and WOMAC pain improved about 50% from baseline in ALL groups, including placebo. It is the cleanest demonstration in the literature that adding measurable cartilage and relieving pain are two different results, and that one does not deliver the other.

4. MACI (autologous cultured chondrocytes on a porcine collagen membrane, Vericel; STN BL 125603) IS an FDA-LICENSED cell therapy - and its approved indication is narrow and specific: repair of symptomatic, single or multiple FULL-THICKNESS cartilage defects OF THE KNEE, with or without bone involvement, in adults. It is not approved for osteoarthritis. The existence of one licensed cartilage cell therapy for focal defects is the sharpest available way to show what a licensed 'regeneration' product actually looks like, and how far it is from an injection for a worn joint.

5. Eighty patients with a single symptomatic chronic femoral condyle cartilage defect were randomized to autologous chondrocyte implantation or microfracture and followed 14-15 years. No significant difference in clinical scores emerged at long-term follow-up; there were 17 failures in the ACI group versus 13 in the microfracture group and more total knee replacements after ACI (6 versus 3). Fifty-seven percent of surviving ACI patients and 48% of surviving microfracture patients had radiographic early osteoarthritis (KL >=2).

6. The SUMMIT randomized trial treated 144 patients (mean age 33.8, mean lesion 4.8 cm2) with at least one symptomatic focal cartilage defect (Outerbridge III/IV, >=3 cm2) of the femoral condyle or trochlea. Matrix-applied characterized autologous cultured chondrocytes (MACI) improved KOOS pain (37.0 to 82.5) and function significantly more than microfracture (pain 35.5 to 70.9) at 2 years, with histological and MRI assessment of the repair tissue. This is what a positive cartilage-repair trial looks like - in young patients with a discrete hole, not a worn joint.

7. A 2025 systematic review of 1,125 patients receiving intra-articular injections for glenohumeral osteoarthritis reported a 7.2% overall complication rate and a 3.2% rate of proceeding to surgery. Hyaluronic acid showed consistent though modest benefit, while the evidence for ORTHOBIOLOGICS (PRP, bone marrow aspirate concentrate, mesenchymal stem cells) 'remains limited', mainly because of heterogeneity in design, outcomes and patient characteristics.

8. The August 2025 AAOS Evidence-Based Clinical Practice Guideline on Management of Rotator Cuff Injuries issued 25 recommendations and 4 consensus statements. Among its updates it clearly RESTRICTS the use of platelet-rich plasma and marrow stimulation in rotator cuff repair, limits prolotherapy in full-thickness tears, establishes CT as an adjunctive imaging modality, and endorses early mobilization after repair of small-to-medium tears.

FDA states plainly that no stem cell, exosome, stromal vascular fraction, umbilical cord blood, Wharton's jelly or amniotic-fluid product has been approved for the treatment of ANY orthopedic condition - it names osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain and shoulder pain individually. The only FDA-approved stem cell products in the United States are cord-blood-derived blood-forming stem cells for disorders of the hematopoietic system, and there are currently no FDA-approved exosome products.
